Dear Parents/Guardians:

The dates and times for fall conferences, grades 1 through 6, are listed below.

Please indicate your preference on this form for all siblings in grades 1 through 6.

Please return the form to school by Wednesday October 19, 2005. You will be receiving a confirmation slip from your child’s teacher as to the date and time of your child’s conference. If you do not receive a confirmation slip by November 4, 2005, please contact your child’s teacher.
